Personalised Care: A Life Well Lived Should Continue to Be Lived

Personalised Care

Customer Case Study June 2026

Background

Every person we support through personalised care has a unique story, shaped by a lifetime of experiences, achievements and passions.

One of our customers has led a particularly fascinating and accomplished life. Originally from Liverpool, she began her career in banking before moving to London to pursue her passion for writing. She spent eight years working as a journalist for a publisher specialising in trade and technical magazines before later establishing and successfully running her own printing company alongside her husband. Following retirement, she continued to write fiction and remains someone who enjoys learning, conversation and discovering new things.

Family has always played a central role in her life. She has two sons, one living in London and the other in America. Following a recent visit from her son from the United States, discussions naturally turned to how she could continue enjoying life to the fullest and experience more opportunities to get out and about.

Together, it was agreed that introducing regular outings could provide greater variety, stimulation and enjoyment, while helping her maintain strong connections with the world around her.

Our Approach & Services Provided

At Braeburn Care, we believe great care extends far beyond practical support within the home. While assistance with daily living remains important, true person-centred care is about understanding what matters to each individual and helping them continue enjoying the life they want to live.

Following conversations with the customer and her family, our Team Leader, Michaela, arranged for regular weekly outings to be incorporated into her support plan. These outings were carefully tailored around her interests and preferences, creating opportunities to explore local attractions, enjoy different environments and spend meaningful time engaging with the community. Since introducing these visits, she has enjoyed exploring a variety of destinations and local places of interest.

One particularly memorable outing involved a visit to the seaside. Accompanied by Micheala, she spent time enjoying the fresh sea air, watching the world go by from the seafront and taking in the atmosphere of a busy coastal town. The day included all the traditional seaside favourites, from fish and chips overlooking the sea to enjoying an ice cream in the sunshine.

The photographs from the trip captured more than simply a day out. They reflected enjoyment, laughter, companionship and the value of continuing to create new memories at every stage of life.

Why Meaningful Activities Matter

As people get older, opportunities to access the community can sometimes become more limited. Reduced confidence, mobility challenges, transport difficulties or the loss of a partner can all impact how often someone gets out and about. Yet maintaining connections, enjoying new experiences and continuing to participate in community life remain incredibly important for overall wellbeing.

Research consistently highlights the positive benefits that meaningful activities can have on both physical and emotional health. Regular outings can help to:

  • Reduce feelings of loneliness and isolation
  • Promote positive mental wellbeing
  • Encourage conversation and social interaction
  • Provide valuable cognitive stimulation
  • Build confidence and independence
  • Create a sense of purpose and enjoyment

Support overall quality of life

For this customer, regular outings have become something to look forward to each week, providing opportunities for new experiences, meaningful conversations and a welcome change of scenery.

Supporting the Individual Behind the Care Plan

One of the principles that sits at the heart of person-centred care is recognising the individual behind the support they receive. A care plan should never define a person. Behind every support package is a lifetime of experiences, relationships, interests and achievements.

For someone who has spent their life building businesses, writing professionally and pursuing creative interests, remaining engaged with the world continues to be hugely important.

By understanding her background and what brings her enjoyment, we have been able to provide support that goes beyond meeting practical needs and instead focuses on enhancing overall quality of life.

The Impact on Family

For families, knowing a loved one is safe is important. Knowing they are happy, engaged and continuing to enjoy life is equally valuable. With one son living in London and the other based overseas, regular communication and reassurance provide significant peace of mind.

Knowing that their mother is enjoying meaningful experiences, maintaining social connections and continuing to explore the world around her gives confidence that she is not simply being cared for, but is continuing to live well.

The Impact of Our Support

  • Safe: Providing support during outings enables customers to access their community safely and confidently.
  • Effective: Regular social interaction, fresh air and community engagement contribute positively to overall wellbeing.
  • Caring: Support is tailored around individual interests, preferences and personal goals.
  • Responsive: Activities are adapted to what matters most to the individual, creating truly person-centred support.
  • Well-Led: Close collaboration between the customer, their family and our care team ensures support remains meaningful and outcome-focused.

The Difference Personalised Care Can Make

At Braeburn Care, we believe care should enable people to continue living the life they choose, not simply manage day-to-day routines. Sometimes that means assistance at home. Sometimes it means support attending appointments. And sometimes it means a trip to the seaside, fish and chips by the coast and the chance to create another treasured memory.

Because a life filled with experiences, interests and achievements does not stop when care begins. With the right support, it can continue to flourish.
